How do people get referred to the service?
The Memory Assessment Service accepts referrals from GPs and other healthcare specialists.
If the referral is from a source other than a GP, the Memory Assessment Service will inform the GP and request a medical history and medication list.
If you, or someone you know have concerns about memory changes, please contact the GP for guidance. The GP will be able to conduct preliminary health checks and can refer to the Memory Assessment Service if required.
Once a referral has been received by the service, a letter will be sent out to acknowledge this. The letter will provide individuals with an estimated waiting time and the details of support that can be accessed whilst they are awaiting an appointment.
