Accessing health records held by Derbyshire Healthcare
The Records Management Team deals with requests for information, both under the Data Protection Act 2018 and the Access to Health Records Act 1990.
Derbyshire Healthcare NHS Foundation Trust primarily focuses upon adults’ and children’s mental health services. For records from the Royal Derby Hospital, please contact the University Hospitals of Derby and Burton NHS Foundation Trust.
If you have been asked to provide supporting documents for a PIP (Personal Independence Payment) application, please note that it may be quicker to request a letter from your current care team or GP. This letter should include your current diagnosis and how this affects you on a daily basis.
We are only able to release information from our Trust, and not from the local authority, education, Police, other NHS Trusts, and private healthcare providers. We typically provide the most recent 24 months’ worth of records, unless there is a specific time period you require. We aim to respond to all requests within 30 days.
